At Unique Health Solutions Inc., we provide personal care services for disabled adults, helping them maintain their independence, quality of life, and to remain in their own homes. Our dedicated caregivers are trained to support patients with their activities of daily living following plans tailored to their individual needs.

Our staff are professional RNs, CNAs, In-Home Aides, and Administrative Staff who are thoroughly trained & experienced to provide compassionate care and support to citizens we serve.

Our RNs create personalized care plans that cater to the specific needs of each individual. They make initial visits to determine a patient's need, then periodic follow-ups to see if the plans work or needs have changed.

PERSONAL CARE SERVICES
TARGETED HUMAN SERVICES
Personal care services are offered at your request or can be initiated by your physician, hospital personnel, community agent, or your family. Services can be available seven days a week as determined by you and a qualified Registered Nurse and as approved by our doctor. If you need help with bathing, dressing, Mobility, or any other activity of daily living, our professional, caring in-home aide can help. Additionally, our nursing staff will check on you regularly as your care and satisfaction is our main concern.
Human Services Practitioner Services provide easier access to care and services if you have a qualifying illness. We will help you identify your needs and goals. Together, we develop a plan to help you meet your goals. We will help you find the care and services in your community. Also, our human services practitioner will assist you with any applications or program requirements. We will always respect your privacy and dignity. Equally important, we will be sensitive to your unique situation, personality, and culture.
