Targeted Human Services
PACE (Program of All-Inclusive Care for the Elderly) and CAP (Community Alternative Program) are programs for disabled adults that support personal care services for persons who need a higher level of care at home. You can talk to your doctor or provider about these services.
Human Services Practitioner Services provide easier access to care and services if you have a qualifying illness. We will help you identify your needs and goals, develop a plan to help you meet your goals and help you find the care and services. Also, our human services practitioner will assist you with any applications or program requirements. We will always respect your privacy and dignity.
Personal care services are offered at your request or can be initiated by your physician, hospital personnel, community agent, or your family. Services can be determined by you and a qualified Registered Nurse and as approved by our doctor.
If you or someone you care about need help with bathing, dressing, eating, mobility, or any other activity of daily living, our professional and caring in-home aide can help. Personal care services can provide an aide to help with daily activities. Once services are established and an aide begins providing care, our nursing staff will check on you regularly because your care and satisfaction is our main concern. Personal Care Services must be requested. The process usually begins with your doctor or primary care provider.
